payment options

Accepted Insurances:

As of March 2021 RLK Eye Associates is the new independent optometry practice located inside the Cumming Costco Warehouse.

RLK Eye Associates currently accepts only specific VISION and MEDICAL insurance plans.
VISION plans cover only routine eye examinations and at times a portion of your contact lens fitting fees (if applicable). You are still responsible for all CO-PAYs and remaining fees that your vision plan does not cover. MEDICAL plans cover all visits pertaining to problems such as a red eye, eye pain, eye infection, dry eyes, diabetic eye exams, flashes/floaters, and glaucoma exams.

We DO NOT submit claims for out-of-network insurance plans. We can provide you with an itemized copy of your receipt and you are responsible for submitting this yourself. RLK Eye Associates guarantees no reimbursement for out-of-network claims, that is between you and your insurance company.

We will be happy to explain all benefits and coverage that pertain to your insurance based on your type of examination.
